Program Description & Educational Objectives
In this activity, experts in insulin-dependent diabetes discuss the optimal management of patients in long-term care settings.
Upon completion of this activity, participants will be able to:
- Demonstrate the ability to translate knowledge of hyperglycemia treatment, including the benefits and risks, evidence-based management strategies, and practice guidelines in the LTC setting
- Improve the ability of LTC practitioners to employ collaborative team care techniques and utilize all available resources to provide consistent clinical practices and improve diabetes care in the LTC population
- Demonstrate improved patient outcomes, representing a reduction/closure of diabetes healthcare performance gaps (A1C, glycemic control, SSI therapy) in the regionally defined care systems
Target Audience
This activity has been designed to meet the educational needs of US physicians, medical directors, physician assistants, consultant pharmacists, nurse practitioners, nurses, and other healthcare professionals who are active in the management of patients with insulin-dependent diabetes.
Requirements for Successful Completion
In order to receive credit, participants must view the activity and complete the post-test and evaluation form. There are no pre-requisites and there is no fee to participate in this activity or to receive CME/CNE/CPE credit. Statements of Credit are awarded upon successful completion of the post-test and evaluation form.
