Program Description & Educational Objectives
In this program, experts provide a concise but comprehensive overview of the current challenges involved in implementing routine HIV screening in primary care practices. Potential barriers to screening implementation that may be present in specific geographic regions will be discussed, and strategies to overcome these barriers will be provided. Practical guidance will be offered for establishing an effective model of coordinated care for patients with HIV, based on understanding of specialty and primary care provider roles.
Upon completion of this activity, participants will be able to:
- Explain the rationale for implementing routine HIV testing and care linkage in accordance with national protocols, and identify issues and potential barriers present in their geographic service area
- Construct strategies to overcome barriers to operationalizing routine HIV testing and linkage of diagnosed patients to specialty care while using practical strategies to foster patient engagement in the care process
- Integrate an effective model of coordinated care for patients with HIV, based on comprehensive understanding of specialty and primary care provider roles and characterized by integration among specialty and primary care clinicians, as well as relevant support services
